Flow is a process and a way of working. This involves a what matters to me conversation with the client to identify goals and ‘team around the person’ meetings (TAP), pulling in organisations that can assist the client on working toward and achieving their goals, wide range of non-medical interventions and client’s only having to tell their story once. For those wishing to purely connect with their community, ‘Community Around the Person’ (CAP) meetings can be held that include the Community developers acting as a conduit to patients accessing the assets of the wider ‘One Community’.
