High Flow (National Lottery)

Project aim: similar model to High Flow HIU and delivery approach. High Flow (NL) supports people with two or more complex needs on a long-term basis through offering the individual a choice in the support they receive. This includes full casework, TAP’s, CAP’s (Community Around the Person) and signposting. This differs from High Flow HIU as the individual does not need to have a high demand on Public Services to receive support.
Referral pathways: open to various organisations/services and self-referrals.
