Introduction to Flow

What is the Flow approach? 

Flow is a way of helping people get the full support they need to live the life they want.  

Flow is about supporting the workforce across the public sector to have the tools to understand what matters to people (not organisations) and the roles needed to ‘pull in’ support from a whole team so that support flows around the person, people don’t bounce around the system, never getting the support they need increasing failure demand.  

Primary Care Flow

Primary Care Flow provides a way of putting person-centred care into practice with a process that supports and provides the workforce with the tools to understand what matters to people and the roles needed to ‘pull in’ support from a whole team so that support flows around the person and people don’t bounce around the system, never getting the support they need and increasing failure demand. The process involves  

  • exploring what matters to the person 
  • identifying the best treatment, care and support 
  • supporting an individual to set goals and thinking about actions they can take to reach them  
  • bringing a Team Around the Person (TAP)
When do I reach out to the Flow team? 
  • Are you feeling stuck with your patient? 
  • Are there multiple barriers that are getting in the way of your patient’s needs?
  • Do you feel there could be multiple agencies and organisations that might be able to help e.g. housing, finance, community-based activities, volunteering etc.
